  1. if its cast iron it will need the part replaced if its steel it can be welded you can temporarily use refractory cement to repair it for a short time
  2. Weld it.
  3. Dont worry about it. I have see fire backs that have been cracked for years.
  4. Personally, I would call my local fireplace shop and discuss it with someone who can answer your question. In my opinion, I'd replace it. I'd be afraid of a fire.
